Procedure

• Filling up 1 declaration form in the prescribed form or written form showing the death registration information. Download here
• Carrying the original Household Registration Book and Identification Card of the dead.
• Getting the original Death Note
• Also bringing the Identification Card or Passport of the people who declare the death.

Required Documents

1. Completed 1 declaration form.
2. In case of necessity the applicants must bring original household registration book or its endorsed copy to collate.
3. Death Note is a must in any cases.
4. Submitting the ID card or passport of the dead.

Office Locations & Contacts

The applicant can call 1080 (The Information Center) to ask the address the People's Committees near where the dead live most and come there to register a death.

Eligibility

• The parents, wife, husband, children, grandchildren or other relatives (over 18years old) of the dead can go through this procedure to obtain a death certificate of that people.
• People's Committees of communes, wards and towns that are the last residence of the dead carry out the death registration.
• In the case of not knowing the last residence of the dead, the People's Committees of communes, wards or towns where they did die carry out the death registration.

Registering a death does not charge a fee

Validity

The validity is ??lifetime.

Processing Time

Time to complete the application for registering a death is only 1 day from the date of receipt of the full dossier as prescribed. However, in case of necessary clarification, the processing time is not exceeding 5 days.

Instructions

• Once there is a death, the relatives of the dead have to report to the competent authorities to get the Death Notice for registration:
  • For the people who died in the hospital or Medical Facility, the hospital director or person in charge of medical facilities that issued Death Notice.
  • For people who reside in one place but died in other places (that is not hospital and Medical Facility), the People's Committees of communes, wards or towns where they died are responsible to issue Death Notice.
  • If the dead are active military personnel, defense employees, military reservists during military training or status check of combat readiness, combat services and those who were gathered on duty military by military units that directly manage, the heads of those units can issue that Death Notice.
  • If the dead were in prisons or the temporary jails, the heads of those agencies can issue Death Notice.
  • If the dead were in the reformatory schools or the educational institutions managed by the Public Security Sector, the heads of those agencies can issue the Death Notice.
  • In the case of the deaths due to the implementation of the death penalty, the Chairman of the Council implementing that death penalty is responsible to issue Death Notice.
• In case there is no death notice you have to submit the paperwork replacement as follow:
  • In case the dead were declared to die by the court, that declaration of the court is legally effective instead of Death Notice.
  • In the case of the dead on the vehicle, the commander or transport controller make records to confirm the death with the signatures of at least two of the vehicles traveling on it. The records confirming die are able to be instead of the Death Notice.
  • If the people who died at home or in residence, the written record confirming the death of the witnesses could be instead of the Death Notice.
• The applicant must submit the Death Notice or the records confirming the death.
• After checking the valid documents, Justice Officials recorded in the death registration book and issue a death certificate. The chairman of People's Committee shall sign and give one original Death Certificate. The copies of the Death Certificate issued according to the request of applicants.

Need for the Document

The documents of registering a death are issued to the relatives of the dead in order to obtain a death certificate for the funeral director and for dealing with the deceased's estate.

Information which might help

• In case the dead has no relatives, the documents of registering a death are issued to the Peoples Committees for doing registration of a death.

• Within 15 days from the date of death, the relatives must go for Register a death. Exceeding that limit time, you will be subject to warning and do the registration procedures for your overdue.

Facilitating the funeral director and for dealing with the deceased's estate.
