ProcedureEdit

  1. Go to the Consul's Office to apply for the procedure.
  2. Present or submit all required documents. A list of documents that must accompany your application are listed below. Note: You may be asked to present additional documents, it will be better if you can give the Consular office a call first before coming in just to make sure that you have all the required documents.
  3. Pay all applicable fees.

Required DocumentsEdit

If the parents are married (possible appearance of a parent) :

  • Statement by the mother or father of the child.
  • Certificate obtained from the medical institution where the child was born (original and photocopy)
  • Passports of the child's parents
  • The marriage certificate of the child's parents (certified copy)


If one of the parents is a citizen of a foreign country:

  • You must provide notarized consent of the father (or mother) of the child citizen


If the parents are unmarried ( in the case of recognition of paternity of a child ) (both parents must present when the application is submitted) :

  • Joint statement of the child's parents to establish paternity (form obtained from the Consul's office)
  • Certificate obtained from the medical institution where the child was born (original and photocopy)
  • Passports of the child's parents


If one of the parents is a citizen of a foreign country:

  • notarized consent of the father (or mother) of the child


If the child is being raised by a single parent (mother), in case of non-recognition of paternity of a child (the mother must be present to submit the application):

  • Statement by the child's mother (installed application form the consul ) .
  • Certificate obtained from the medical institution where the child was born (original and photocopy)
  • Passport of the child's mother .
