Procedure

Apply Online

1. To Register as Health Worker, the applicant has to make the application online via link
2. The applicant has to enter the username & password and click “Login”.
3. The applicant has to select the “Health Worker” option from the list.
4. Enter all the details in the application form and click the “Next” button.
5. Also, upload all the necessary documents which are mentioned in the “Required Documents” section of this page.
6. Pay the prescribed amount fee in online mode.
7. After completing all the details, the applicant has to click the “Submit” button.
8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
9. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
10. If required, the applicant has to visit the office to complete an additional process for checking the health.
11. Once the process gets completed, the applicant can download the certificate through the online portal.
12. This entire process can be completed within 15 days or as per the timeline advised by the respective department authority.
