If your permit is lost or stolen, As a first step you would have to contact your local police station where you have to register a FIR which has to be submitted along with your application.

Make sure you have all the necessary documents
FIR Copy - A certificate to the effect that the permit was lost or non-traceable issued by the Station House Officer concerned should be submitted. If the permit is damaged one, then the remains can be submitted and FIR is not required.
Application forms can be obtained from the appropriate RTO offices that should be completed and presented along with documents and appropriate fees. Officials would start processing your application.
After checking the documents and vehicle for its correctness and other appropriate requirements the application will be processed.

Need for the DocumentEdit

Contract carriage Motor Cab Permit is needed for an tourist vehicle operator to carry passengers on a hire / rental basis from one place to another
Motor cab means any motor vehicle constructed or adapted to carry not more than six passengers, excluding the driver for hire.
One cannot operate his motor cab without this permit in the allowed route.
If the permit is lost, stolen, damaged or misplaced then one need to apply for a duplicate permit for the Vehicle
