If your permit is lost or stolen, as a first step you would have to contact your local police station where you have to register a FIR which has to be submitted along with your application.

Make sure you have all the necessary documents
FIR Copy - A certificate to the effect that the permit was lost or non-traceable issued by the Police Station concerned should be submitted. If the permit is damaged one, then the remains can be submitted and FIR is not required.

If the permit is lost, stolen, damaged or misplaced then one need to apply for a duplicate permit for the vehicle.
Maxi Cab Permit is needed for an tourist vehicle operator to carry passengers on a hire / rental basis from one place to another.
Maxi cab means any motor vehicle constructed or adapted to carry more than six passengers, but not more than twelve passengers, excluding the driver for hire.
