Apply In-Person:

  1. To Obtain FSSAI license for Co-operative Companies, You have to approach the Commissioner of Food Safety / Officer-In-Charge for Food Safety.

  3. You can download the application Form-B from the following link: Form B.
  4. Please make sure you are eligible to make the application by going through the eligibility criteria provided under the “Eligibility” section of this page.
  5. First download the Form-B from the link and fill all the required information and you need to submit the form to the Commissioner of Food safety department. The address is mentioned in the office locations & contacts section below.
  6. Please complete the application form and make sure you have all the documents that are listed under “Required Documents” section of this page.
  7. Pay the appropriate fees and submit the form to the Officer-In-Charge in the Food Safety department.
  8. Now the authorities will verify your application and validate the submitted documents.
  9. Authorities will inspect the facilities that are related to the Co-operative Companies and if they are satisfied with the standards followed the authorities will provide the franchise permission.
  10. The applicant will be later requested to collect the FSSAI license for the Co-operative Companies on a specified date.



Apply through mobile app:

  1. The applicant willing to Apply FSSAI License must download the ‘’Food Safety Connect’’ mobile app.
  2. The applicant can download the app from play store using the following Link: link
  3. Once downloaded, the applicant must click the button “Apply for license” in the main page.
  4. Then the applicant has to select their respective state from the dropdown menu where premises is located for which License/Registration will be obtained.
  5. Next select the type of Business, Now Select the annual turnover of the business. After that Click on the “Proceed” tab.
  6. On the next page, click on the “Click here to apply Central License for all Businesses”.
  7. Next Form-B page will be displayed, fill all the required information under premises details section and Click on the “Save & Next” tab.
  8. On the Production selection page, fill all the details for DGFT Authentication and Click on the “Proceed with DGFT validation” to authenticate the details.
  9. Next Click on the “Save” to save the authenticated details. After Successfully Proceeding with DGFT validation, click on the “Save & Next”.
  10. Next fill all the Communication details and Click on the “Save & Next”.
  11. Under required documents section upload all the relevant documents and Click on the “Save & Next”.
  12. Next pay the fees and submit the application.
  13. After Successful Completing the payment, a receipt will be generated with a 17-digit reference number make a print of it and keep it safe for future reference.
  14. You can track the application through the home page.
  15. This application will be processed by them and they will notify you through your e-mail or by SMS.

  1. ID proof and Address proof
  2. Identity Proof Documents such as Ration Card, Department Issued ID, PAN Card, Passport, Senior Citizen Card, Aadhaar Card, Driving License, Voter ID Card
  3. Affidavit
  4. Declaration of Food Safety Management Plan
  5. List of food categories
  6. No Objection Certificate (NOC) from the owners or the partners
  7. Medical Certificates of Employees
  8. Online application form
  9. Certificate by District Central Cooperative Bank regarding Bank Balance (
  10. List of members/promoters with contribution towards share capital & admission fee
  11. PAN Card of all the members (minimum 10 members/families – number varies with state to state) of the proposed Companies has to be submitted along with the application.
  12. Two Passport Size photos.
  13. Document evidencing payment of fee.
  14. Memorandum and Articles of Association.
  15. No objection letters from directors as applicable
  16. Certificate by Police Station stating that the Main Promoter and members are not involved in criminal activity (As per requirement)
  17. Payment of stamp duty
  18. Form.
