Procedure

Apply Online

1. Visit the official Canadian government veterans service page, use the following link.
2. Click on “Proceed to application form”.
3. The applicant will have to register an account.
4. Enter details such as email, username and password and click on “Register”.
5. The applicant will be directed to another page showing their profile.
6. Enter first name and last name.
7. Select mode of communication.
8. Click on “Update”.
9. The applicant will be directed to another page where they will have to enter details such as given name, surname, ID details, SIN, and so on.
10. Click on “Next” once all the details have been provided.
11. The applicant will be directed to another page where they will have to upload a passport size photo, ID proof and a photo of their Canadian armed service certificate.
12. Click on “Submit”.
13. The application will be submitted for processing and the applicant will receive their card within 90 days from the date of application by registered post.

Apply by Mail

1. The applicant will have to download the application form from the following Application form.
2. Fill in all relevant details in block letters only.
3. The applicant is advised to keep a copy of the completed application form for their own records.
4. Attach all documents as mentioned under the “Required Documents” section of this page.
5. Mail the application form along with scanned/printed copies of all documents to this address: Director General Defence Security National Defence Identification Service Veteran’s Service Card National Defence Headquarters MGen George R. Pearkes Building 101 Colonel By Drive Ottawa, ON K1A 0K2.
6. The applicant will receive their card within 90 days from the date of submission of the application, by registered post.

• A passport-like quality photo (Photo requirements can be found at the bottom of the application form)
• Provincial Driver’s license
• Provincial health card
• Aboriginal status card
• Passport
• NDI-21 (DND civilian card)
• Scanned copy of ID such as a government-issued ID.

Eligibility

• The applicant should have completed basic training.
• The applicant should have been honourably released from the Canadian Armed Forces.

Fees

• The applicant does not need to pay fees.

Validity

• The card does not need to be renewed as it is valid forever.

Need for the Document

• The VAC health card can be used to avail medical services for the applicant.
• The new Veteran’s Service Card helps the applicant stay connected to military and veteran support programs during the applicant’s transition from military to civilian life.
• This card also recognizes the applicant’s service as a Canadian Armed Forces (CAF) member.
