Apply Online
- Visit the official Canadian government veterans service page, use the following link.
- Click on “Proceed to application form”.
- The applicant will have to register an account.
- Enter details such as email, username and password and click on “Register”.
- The applicant will be directed to another page showing their profile.
- Enter first name and last name.
- Select mode of communication.
- Click on “Update”.
- The applicant will be directed to another page where they will have to enter details such as given name, surname, ID details, SIN, and so on.
- Click on “Next” once all the details have been provided.
- The applicant will be directed to another page where they will have to upload a passport size photo, ID proof and a photo of their Canadian armed service certificate.
- Click on “Submit”.
- The application will be submitted for processing and the applicant will receive their card within 90 days from the date of application by registered post.
Apply by Mail
- The applicant will have to download the application form from the following Application form.
- Fill in all relevant details in block letters only.
- The applicant is advised to keep a copy of the completed application form for their own records.
- Attach all documents as mentioned under the “Required Documents” section of this page.
- Mail the application form along with scanned/printed copies of all documents to this address: Director General Defence Security National Defence Identification Service Veteran’s Service Card National Defence Headquarters MGen George R. Pearkes Building 101 Colonel By Drive Ottawa, ON K1A 0K2.
- The applicant will receive their card within 90 days from the date of submission of the application, by registered post.

- Provincial Driver’s license
- Provincial health card
- Aboriginal status card
- Passport
- NDI-21 (DND civilian card)
- Scanned copy of ID such as a government-issued ID.

Need for the DocumentEdit
- The VAC health card can be used to avail medical services for the applicant.
- The new Veteran’s Service Card helps the applicant stay connected to military and veteran support programs during the applicant’s transition from military to civilian life.
- This card also recognizes the applicant’s service as a Canadian Armed Forces (CAF) member.
