Submitting of ApplicationEdit

Receiving of the CertificateEdit

Documents are mailed to the address of the person who is making the application. It is advisable to contact a Registry Agent or Registry Connect for special arrangements and/or rush services options.

EligibilityEdit

  • The executor of the deceaseds persons estate. A copy of the will identifying the executor must be attached to this application.
  • An adult next-of-kin* of the deceased person including a minor parent, spouse or partner.Proof of relationship or an Affidavit of Relationship must be attached to the application.
  • A guardian or trustee, for the deceased person immediately before death as established by court documents. A copy of the legal document must be attached to the application.
  • A person with an order from the court. A copy of the order must be attached to the application.
  • A joint tenant with the deceased immediately before the deceaseds death. Certified copy of the title to property showing joint tenancy must be attached to the application.
  • A person who is a designated agent for a person described in the first to third eligibility. Consent and the statutory declaration in the application must be completed.
  • An adult relative of the deceased person when there is no person eligible to apply as described in the first to fourth eligibility. Proof of relationship or an Affidavit of Relationship must be attached to the application.
  • A funeral home representative who is making arrangements for the deceased person. Proof of occupation must be attached to the application.
  • A lawyer for the person described in the first to fifth, or seventh eligibility. A copy of a valid Law Society card must be attached to the application.

Anyone may apply for a death document when:

  • The death record is 50 years or older.

In addition to the government certificate fee, registry agents are authorized to charge a service fee.

Processing TimeEdit

Once a registry agent submits a request or the Vital Statistics office receives an application, it is usually processed within two to three business days provided there is no additional information required.

Need for the DocumentEdit

Alberta Vital Statistics maintains a record of all registered deaths that occur in Alberta and can only issue certificates or documents for those particular events. Death certificates are issued using the information from the original Registration of Death, completed at the time of death. If a record cannot be found, a record search can be completed for a consecutive three-year period.

  • Kind of Death Documents Available to Order:
    • Certified large size Certificate of Death
    • Photocopy of a Registration of Death
    • Photocopy of a Medical Certificate of Death
    • Certified small size death certificates are not available
