Apply In-Person:

  1. To apply for a change of name in Alberta, the applicant will need to visit the nearest authorised registry agent. The applicant can find the agent here: registry agent. The applicant can find the nearest agent using a map view, list view or by manually typing the city/village they live in.
  2. The applicant will need to refer to the information guide before filling out the application guide. (The information guide is in the application form)
  3. The applicant will be given a form to fill out, fill this in and make sure all the details are filled in, correctly.
  4. The link to the application guide can be found under the “Required Documents” section of this page.
  5. Or use the following link to the application form.
  6. Carry all supporting documents. The applicant can get further information from the registry agent or check under the “Documents to Use” section of this page.
  7. Applicants aged 12 and older will also need electronic fingerprints. Visit the nearest fingerprinting agency to give biometric information.
  8. Find a fingerprinting agency under the “Information which might help” section of this page.
  9. The applicant will receive a confirmation mail.
  10. The applicant has to submit the application along with the confirmation mail.
  11. Applicant’s aged 18 or older will have to submit any criminal records.
  12. The applicant will also have to give birth records that have their old name on them.
  13. Once all documents have been submitted to the agent, they will forward them for further processing.
  14. Once submitted and approved, the applicant will receive a certificate with their changed name through regular mail, within a few days to a week.

    • Passport
    • Defence personnel ID
    • Driver’s license
  • Proof of name
  • Proof of all previous legal changes of the name outside Alberta (if any)

  • A criminal record check or police information check

    • Proof of marital status
    • Proof of relationship if you’re changing the name of a child/ward

EligibilityEdit

  • The applicant is a resident of Alberta.
  • The applicant must remain a resident until the change of name document has been made.
  • The applicant must be 18 years of age or older.
  • A person designated as a dangerous offender cannot change their name.
  • A person designated as a long-term offender cannot change their name.
  • A person designated as a high-risk offender cannot change their name.
  • If the applicant is under 18 years, they can apply under special conditions, these are:
  • If the applicant is changing the name of a minor but the minor turns 18 before the application is processed, in which case, a new application will need to be submitted.
  • The applicant needs to make sure there is enough time to process the application before submitting an application to a registry agent.


FeesEdit

  • If the applicant is reclaiming their indigenous name, they can do so free of cost.
  • The applicant will have to pay a fee of 120$ for each name change document.
  • The applicant will also have to pay a registry agent service fee.
  • The applicant will need to pay a fee for fingerprinting.
  • The applicant will have to pay fees for a criminal record check.

The document is valid forever and does not need to be renewed.

Processing TimeEdit

The applicant shall receive their name change document within a few days to a week from the submission of the application.
