Required DocumentsEdit

Legal name & date of birth (primary document)

The following documents are acceptable for proving legal name and date of birth. If your legal name is different than what is printed on your birth certificate, you will need to bring in the document(s) that provide proof of the name change, unless you have previously obtained an Alaska learner's permit or Alaska State ID card in your current name.

  • An original or certified copy of a U.S. birth certificate (From Vital Statistics)
  • If you were born in Puerto Rico, click here for important information
  • Passport (Issued by the United States or Canada. Must be current.)
  • An Alaska learner's permit or Alaska State Identification card.
  • Military Identification for active duty, retiree or reservist.
  • Certificate of Citizenship, Naturalization or Birth Abroad.
  • Passport issued from another country, with appropriate immigration documents.
Name Change

The document provided must be an original or certified copy of the original issued by a government agency.

  • U.S. Passport (Must be current)
  • Amended birth certificate
  • Certified adoption papers, marriage certificate, court order or divorce decree
  • Certificate of Dissolution, Divorce or Annulment
  • Certificate of Naturalization
  • Certificate of Name Change
Social Security Card
  • Original Social Security Card ( Metal and laminated cards are not acceptable).
  • Letter from the Social Security Administration verifying your name and number.
  • Letter from the Social Security Administration verifying you have never been assigned a social security number.

Residence Address

The following items are acceptable for proving residency. The document must show your residence address, not a post office box or other mail cache facility.

  • Utility bill
  • Alaska voter registration card
  • Alaska title and/or registration (Issued at least 30 days prior to application)
  • Paycheck Stub
  • Medical Assistance card
  • Public Assistance card
  • Cancelled check or bank statement
  • Mortgage or rental documents

EligibilityEdit

An applicant for an Alaska provisional driver license must be at least 16 years of age and have held a valid driving permit for a minimum of 6 months.

Note: Please be aware that the Alaska DMV does not accept non-personalized checks. Your name and address must be pre-printed on any check to be acceptable.



ValidityEdit

A driver license issued to an applicant under the age of 21 will expire 90 days after the applicant's 21st birthday. All other Alaska driver license's expire on the applicants birthday and are valid for approximately five years.

InstructionsEdit

  • You must be at least 16 years of age and have held a valid driving permit for a minimum of 6 months.
  • You must have held a valid instruction permit for a minimum of six (6) months before you can apply for a provisional driver license.
  • While practicing with your valid instruction permit, the adult with you must have been licensed for 1 year and must be 21 years of age or older.
  • If you receive traffic citations which total 6 or more points in a 12 month period or 9 or more points in a 24 month period, you must take a nationally certified defensive driving course. If you do not take a course, the DMV can suspend your privilege to drive.
  • Your parent, legal guardian or employer must provide proof that you have had at least forty (40) hours of driving experience. This must include at least ten (10) hours of driving in challenging circumstances such as inclement weather and nighttime driving.
  • If you are convicted of a violation of a traffic law, you must wait for six months before you can apply for your provisional driver license.
  • A ROAD TEST is required to obtain a provisional license. Once you obtain your provisional license, you must wait for at least six months in order to graduate to a regular driver license.
  • If you are convicted of violating a traffic law or of violating AS 04.16.050 (c) (repeat minor consuming alcohol) while holding a provisional driver license, you must wait an additional six months from the conviction date before you would be eligible for a regular driver license.
  • A new parental consent form (signed in front of a DMV representative or notary public) must be submitted in order to receive a regular license.
  • Parental consent is required to graduate from a provisional license to a regular license.
  • To remove a provisional restriction, you must go to a DMV office. The fee for a duplicate license is charged to remove the restriction.
Provisional License Restrictions (Minimum of six months - up to the age of 18)
  • You may NOT carry passengers under the age of 21, except siblings.
  • You may NOT operate a motor vehicle between the hours of 1:00 a.m. and 5:00 a.m.
Exceptions
  • Unless accompanied by a person who is at least 21 years of age that is licensed to drive the type or class of vehicle being driven.
  • Unless driving to or from your place of employment or within the scope of your employment. All driving must be along the most direct route.

NOTE: Once you reach the age of 18, these restrictions no longer apply to you. You are not required to obtain a regular license at that time. If you choose to obtain a regular license at that time, you would be subject to the vision test and replacement license fees.

Failure to follow Restrictions
  • A violation of the restrictions is an infraction with a $200.00 penalty.
  • Driving in violation of a restricted license under [AS 28.15.057(b)] is a 2 point violation that will appear on the driving record.
