  • A driver's license or official identification card(Issued by another state in the U.S., jurisdiction or territory, unexpired, or expired for not more than one year)
  • An unexpired U.S. or foreign passport.
  • U.S. military identification, military dependent identification or veteran's identification.
  • If you are currently living in Alaska, a BIA or tribal identification card will also be accepted.

Parents listed on the record are eligible to request the certified copies of a certificate of Fetal death.



FeesEdit

Certified copy: $20.00 each

InstructionsEdit

  • A named parent must submit an application and a photocopy of a valid government issued identification, such as drivers license, military ID, passport, or state issued ID.
  • This documentation is required to determine their eligibility in obtaining the Certificate.
  • The certificate can only be issued from the Juneau office. You can send the request by mail with check or money order.
  • The check or money are payable to "Bureau of Vital Statistics ".
