- A driver's license or official identification card(Issued by another state in the U.S., jurisdiction or territory, unexpired, or expired for not more than one year)
- An unexpired U.S. or foreign passport.
- U.S. military identification, military dependent identification or veteran's identification.
- If you are currently living in Alaska, a BIA or tribal identification card will also be accepted.

- A named parent must submit an application and a photocopy of a valid government issued identification, such as drivers license, military ID, passport, or state issued ID.
- This documentation is required to determine their eligibility in obtaining the Certificate.
- The certificate can only be issued from the Juneau office. You can send the request by mail with check or money order.
- The check or money are payable to "Bureau of Vital Statistics ".
