Apply In Person

  1. To Report about Unsatisfactory Equipment, the applicant has to approach the U.S Food & Drug Administration link
  2. If the applicant is a health professional the applicant has to download the form from the following link
    NOTE: Scroll down and click the link to download the pdf.
  3. If the applicant is a consumer the applicant has to download and complete the application form from the following link
  4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit the application form and the documents to the authority officer.
  7. The authority will check the reports, and equipment.
  8. The fee for this procedure has to be paid as per the authority request.
  9. Once all the verification is done by the authority, the applicant receives a notification through a mail.
  10. The applicant can get the money refund or new equipment’s by visiting the office.
  11. The process is done within a month.

  2. If the applicant is a health professional or consumer, select the respective icons.
  3. Once it has selected, the new link page will be opened.
  4. The applicant has to give all the details about the problem and click the “Save & Exit” button.
  5. After this continue with all the information about device, product, patient, reporter.
  6. Finally, review the given detail and click the “Submit” button.
  7. Once submitted, the respective authority will review the information and come for verification process.
  8. Upon verification, the applicant receives a notification through a mail.
  9. The applicant can get the money refund or new equipment’s by visiting the office.
  10. The process is done within 2 months.
