Apply In-Person:
- The applicant has to reach out to local social security office for the application. Contact details are mentioned in “Office Locations & Contacts” of this page or here is the Contact Link
- The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
- Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
- The fee for this procedure has been mentioned under the “Fees” section of this page.
- Concerned authorities will look into the application form and submitted documents before providing the card.
- Acknowledgement letter will be provided within 30 days.
- If the acknowledgement letter has not been given within 30 days, the applicant can visit the office to know the status of their application.

- If the applicant is an existing user, they can log in directly using their “Username” and “Password”.
- If the applicant is a new user, they have to register first by clicking on the “Create an account” option.
- The applicant has to provide their own email address when they create an account.
- While registering, the applicant has to provide all necessary information in the application form and finally click on the “Create an account” button.
- Once the registration is successful, the applicant can log in with their “Username” and “Password”.
- After login, select the “Replacement Documents” tab, and select “Mail my replacement Medicare Card”.
- The applicant must provide all information necessary for the application procedure online.
- The fee for this procedure has been mentioned under the “Fees” section of this page.
- Concerned authorities will look into the application form and submitted documents before providing the card.
- Card will be provided within 30 days through mail.
- If the card has not been given within 30 days, the applicant can call 1-800-772-1213 (TTY 1-800-325-0778) to know the status of their application.
