Sending request via mail

1. Download Application form online. You may not request two different types of certificates using the same form.
2. Prepare your payment, which may be in the form of a check or money order payable to "State Board of Health".
3. Send the completed form along with your payment to:

Obtaining certificate in person

1. Go to any county health department in the State of Alabama to obtain a certified copy of an Alabama marriage certificate. Most marriage certificates can be issued while you wait.
2. Complete a request form and pay all necessary fees.

Eligibility

Alabama law states that marriage certificates are unrestricted. They may be requested by anyone who can provide enough information to search for the record and pay the appropriate fee.

Fees

• Fee to search for a marriage certificate: $15.00 (includes one certified copy of the marriage record or a "Certificate of Failure to Find")
• Additional copies: $6.00 each

Instructions

The Alabama Center for Health Statistics keeps marriage records filed in Alabama beginning 1936. For marriages prior to 1936, certificates must be obtained from the probate office in the county where the marriage license was issued.
