Receiving of the CertificateEdit

Documents are mailed to the address of the person who is making the application. It is advisable to contact a Registry Agent or Registry Connect for special arrangements and/or rush services options.

EligibilityEdit

  • An adult next-of-kin* of the stillborn child, including a minor who is a parent of the child. Proof of relationship or an Affidavit of Relationship must be attached to the application.
  • An adult relative of the stillborn child when there are no living persons described in the first eligibility. Proof of relationship or an Affidavit of Relationship must be attached to the application.
  • A person who is a designated agent for a person described in the first or second eligibility. Consent and the statutory declaration on the application must be completed.
  • A person with an order from the court. A copy of the order must be attached to this application.
  • A funeral home representative who is making arrangements for the stillborn child. Proof of occupation must be attached to the application.
  • A lawyer for a person described in the first, third or fourth eligibility. A copy of a valid Law Society card must be attached to the application.

Anyone may apply for a stillbirth document when:

  • The stillbirth record is 75 years or older.



FeesEdit

In addition to the government certificate fee, registry agents are authorized to charge a service fee.

Processing TimeEdit

Once a registry agent submits a request or the Vital Statistics office receives an application, it is usually processed within two to three business days provided there is no additional information required.

Need for the DocumentEdit

Alberta Vital Statistics maintains a record of all registered stillbirths that occur in Alberta and can only issue stillbirth documents for those particular events. If a record cannot be found, a record search can be completed for a consecutive three-year period. As a courtesy, Vital Statistics issues a document known as a Certificate of Registration in lieu of a Stillbirth certificate.

  • Photocopy of a Registration of Stillbirth: This is a photocopy of the registration completed at the time of the stillbirth. This document contains pertinent information about the details of the stillbirth and provides parental information. It is printed on 8 1/2 x 11 inch paper with an official raised seal and red stamp on the back indicating that it is a certified true copy of the original registration.
