Apply In-Person:

  1. To file Partnership Income in person, the applicant has to visit the office of Canada Revenue Agency that is nearest to their location.
  2. Address and contact details can be found in the following Contact link
  3. Visit the office as applicable and obtain the form T5013 - Statement of Partnership Income from the relevant department.
  4. Application form download links are also available below:
  5. Complete the application form with all the required details and information in the respective fields.
  6. After that, the applicant should attach the documents listed in the “Required Documents” section of this page, with the completed form.
  7. Then, submit the application form and required documents to the relevant official at the office. The application process is free.
  8. On receiving the completed form and the required information, the respective official will assess the submitted information and proceed to file the Partnership Income.
