Procedure

1. Go to the Civil Registry office to register the death of a child.
2. Present and submit required documents.
3. Complete an registration form.

Required Documents

• application (template can be obtained from the Ministry of Justice of the Republic of Armenia, http://www.moj.am address or Civilian Authority)
• an organization or a medical practitioner certifying the death, (document form)
• court's final judgment, if approved by the death of a person, or a person who has been declared dead by the court.
