Procedure

1. Go to the RMV Office
2. Completely fill up the prescribed form
3. Provide the following details

• Social security number
• License
• Birthday
• And digital photo.

Apply Online

1. Go to the website, specifically in the Online license replacement page.
2. Type your social security number, license, birthday and digital photo.
3. Pay the $25.00 using a valid credit card.
