Apply for a personalized license plate in person at a local Alaska Department of Motor Vehicles office location.

Online
  • Online at the Alaska DMV website.
    • The Alaska DMV recommends submitting your personalized license plate online when possible to save on government administrative costs.
By Mail
  • You may also download an application online and mail the complete application to the DMV in Anchorage.

Legal name & date of birth (primary document)

The following documents are acceptable for proving legal name and date of birth. If your legal name is different than what is printed on your birth certificate, you will need to bring in the document(s) that provide proof of the name change, unless you have previously obtained an Alaska learner's permit or Alaska State ID card in your current name.

  • An original or certified copy of a U.S. birth certificate (From Vital Statistics)
  • If you were born in Puerto Rico, click here for important information
  • Passport (Issued by the United States or Canada. Must be current.)
  • An Alaska learner's permit or Alaska State Identification card.
  • Military Identification for active duty, retiree or reservist.
  • Certificate of Citizenship, Naturalization or Birth Abroad.
  • Passport issued from another country, with appropriate immigration documents.
Name Change

The document provided must be an original or certified copy of the original issued by a government agency.

  • U.S. Passport (Must be current)
  • Amended birth certificate
  • Certified adoption papers, marriage certificate, court order or divorce decree
  • Certificate of Dissolution, Divorce or Annulment
  • Certificate of Naturalization
  • Certificate of Name Change
Social Security Card
  • Original Social Security Card ( Metal and laminated cards are not acceptable).
  • Letter from the Social Security Administration verifying your name and number.
  • Letter from the Social Security Administration verifying you have never been assigned a social security number.

Residence Address

The following items are acceptable for proving residency. The document must show your residence address, not a post office box or other mail cache facility.

  • Utility bill
  • Alaska voter registration card
  • Alaska title and/or registration (Issued at least 30 days prior to application)
  • Paycheck Stub
  • Medical Assistance card
  • Public Assistance card
  • Cancelled check or bank statement
  • Mortgage or rental documents

EligibilityEdit

  • You are eligible to order personalized plates if the vehicle is in your name with valid unexpired registration.
  • Personalized plates cannot be ordered for vehicles with expired registration. If your registration is due to expire soon, take the steps to renew now so that your registration is current when your new plates arrive. If you renew your registration, be sure to place the new year tab on the your rear license plate. (A new month tab and year tab will be sent with your new plates.)
  • If your registration expires after ordering the plates, they cannot be issued until the registration is renewed.
  • The vehicle must be in your name before you can request the plates. We cannot order plates for vehicles with temporary permits.
  • If you intend to sell your vehicle soon, please do not order plates for that vehicle.

InstructionsEdit

  • Choose what you want your personalized license plate to display. The plate must be more than 1 letter and up to 6 characters, using some combination of letters, numbers and spaces. Each space counts as part of the 6-character combination.
    • Do not use special symbols such as the percentage, pound or ampersand sign. Radio call signs or anything considered indecent are also not allowed. The message will appear centered on the license plate.
    • To distinguish between the number zero and the letter 0 on your application, put a slash through the zero.
    • The personalized message you choose cannot duplicate a plate that has already been printed. You can list up to 5 messages on your application in case your first few choices aren't available.
  • Apply for a personalized license plate in person at a local Alaska Department of Motor Vehicles office location or online at the Alaska DMV website. You may also download an application online and mail it to the DMV in Anchorage.
    • The Alaska DMV recommends submitting your personalized license plate online when possible to save on government administrative costs.
  • Keep your vehicle registration card handy as you fill out the application. You'll need to refer to your registration renewal number or have your current license plate number and last 2 numbers of your vehicle's serial number
  • Include the appropriate fee and a copy of your current registration with your personalized plate application. You may also include an address change form, if appropriate, and an 830 Form and FCC license if you're requesting HAM radio operator plates.
  • Your address must be correct on your vehicle registration. If it is not, please E-mail ALASKA DMV and advise of the new address. We will need your current license plate number.
  • You may be eligible to update your address online.
