Apply In-Person:
- A complaint must be submitted by the complainant in person/ by an agent to the Chief Commissioner for Persons with Disabilities/ be sent by registered post, addressed to the Chief Commissioner. Find contacts to the Commissioner for Persons with Disabilities in Uttar Pradesh in the provided Link, Link. Follow the "Office Locations & Contacts" section of this page for more information.
- The applicant must write an application on a blank paper, stating the facts relating to the complaint as to when and where it happened.
- The applicant must produce all the documents in support of the allegations included in the complaint. Make sure to have all the necessary documents provided under the “Required Documents” section of this page.
- The Chief Commissioner on receipt of a complaint shall pertain a copy of the complaint to the opposite party/parties referred to in the complaint expecting him/her to provide their version of the case within a certain time. Find out more about that in the "Validity" section of this page.
- During the date of hearing, the parties or their agents must appear before the Chief Commissioner on that date.
- If the complainant/agent fails to appear before the Chief Commissioner on such dates, the authority in his discretion, may either reject the complaint or decide it on merits.
- If the opposite party/ agent fails to appear before the Chief Commissioner on the given date, the authority may take necessary action under section 63 of the Act.
- The Chief Commissioner is authorised to dispose of the complaint ex parte, if necessary at any period.
- The Chief Commissioner may also, at any stage of the proceedings, suspend the hearing of the complaint.
- The complaint shall be resolved within a particular period from the date of notice obtained by the opposite party. The time required for the process is maybe 3 months.
NOTE:
- If needed the parties may be heard in person.

Required DocumentsEdit
- The applicant must provide documents in support of the allegations enclosed in the complaint.
- The applicant must enclose a copy of the Disability Certificate from a valid authority or a copy of the Unique ID for Person with Disabilities issued by the Government of India.

EligibilityEdit
- Must be a Person with Disabilities (not less than 40%).
- Must register the complaint with the Commissioner for Person with Disabilities.

- The opposite party/parties must produce their statement of the case within 30 days or an extended period that is not exceeding 15 days as may be granted by the Chief Commissioner.

- The applicant must file the complaint along with the disability certificate and other required documents as referred in Rule 42 of the Persons with Disabilities (Equal Opportunities, Protection of Rights and Full Participation) PwD Rules, 1996 Link by post or by directly visiting the office.

- Brief description of the nature of disability of the applicant.

Need for the DocumentEdit
- Based on the complaints received, the Department recognizes the problem areas and then the complaints are sent to the concerned officials who are dealing with that category area, associated with the grievance for redress.
Information which might helpEdit
- In case of any infringement of the rights, as prescribed in the Equal Opportunities, Protection of Rights and Full Participation Act, 1995, people with disabilities may make the application from the Chief Commissioner for Persons with Disabilities in the Centre or the State.
- Every Government institution must appoint a Grievance Redressal Officer for section 19 and shall inform the Chief Commissioner or the State Commissioner about the assignment of such an officer.
- Any person, aggrieved with the non-compliance of the conditions put forth in section 20, may file a complaint with the Grievance Redressal Officer, who shall examine it and take up the matter with the association for remedial action.
- The Grievance Redressal Officer maintains a register of complaints in the manner as may be prescribed by the Central Government, and every complaint is inquired within two weeks of its registration and resolved in about 3 months.
- If the aggrieved person is not convinced with the action taken on his/her complaint, he/she may approach the District-Level Committee on disability.
