ProcedureEdit
- Registration shall be conducted at the place of the child's birth or at the place of residence of the parents.
- Application for registration may be done verbally or in writing.
- Submit the application along with all other required documents.
Required DocumentsEdit
- Passports or passport documents authenticating the parents identity (or one of parents). If the document authenticating one of parents can not be presented for a valid reasons, the body of state registration of civil status acts has no right to refuse in the state registration of childs birth. In this case, Information on the second parent will be based on the Marriage Certificate.
- Passport or passport document authenticating the identity of the applicant provided the state birth registration is conducted by another person, who is not a parent.
- Document establishing the paternity of the child: (any of the following)
- marriage certificate
- mothers application joint application of childs mother and father).
If the marriage certificate is not available, the registered marriage can be confirmed with an indication on its state registration in the passports or passport documents of childs mother and father (item 12 of Chapter 1 of Section ??? of the Rules of State Registration of Civil Status Acts in Ukraine).
Documents that will serve as the basis for state birth registration:
- medical birth certificate of form No. 103/? approved by the Decree of the Ministry of Health of Ukraine No.545 of August 8, 2006 registered with the Ministry of Justice of Ukraine under No. 1150/13024 of October 25, 2006, which is issued by the health protection maternity institutions irrespective of subordination and ownership form. If the child was born outside the health protection institution, state birth registration is conducted on the basis of the medical birth certificate or a medical notice of form No. 103-1/? on the health-care institutions supervision over the child, which form is approved by the Decree of the Ministry of Health of Ukraine No.545 of August 8, 2006 registered with the Ministry of Justice of Ukraine under No. 1150/13024 of October 25, 2006.
- medical birth certificate, medical notice on the health-care institutions supervision over the child. (for child who is 1 year and older)
- act completed by relevant authorities(ship captain, commanding officer, train master, etc.) certified by two witnesses and a doctor or a paramedic (if a doctor or paramedic were on the transportation vehicle), in case of childs birth on the sea-, river-, air-vessel, in the train, or in any other transportation vehicle. If a doctor or a paramedic are not available, state birth registration is conducted on the basis of the indicated act and a medical notice on of form No. 103-1/? on the health-care institutions supervision over the child;
- medical certificate of form 106-2/0 on perinatal death approved by the Decree of the Ministry of Health of Ukraine No. 545 of August 8, 2006 registered with the Ministry of Justice of Ukraine under No. 1150/13024 of October 25, 2006, in case of deadbirth.
These documents are to be submitted to the body of state registration of civil status acts for birth registration. In case of lack of grounds for state birth registration specified by this item, state birth registration is conducted on the basis of courts decision on establishing the fact of giving birth by such woman (item 2 of Chapter 1 of Section ??? of the Rules of State Registration of Civil Status Acts in Ukraine).

EligibilityEdit
- Registration may be done by the parents of the child.
- If the parents die or are unable, for valid reasons, to register the childs birth, the following may apply for the registration:
- relatives
- other individuals who has the responsibility to register the childs birth
- authorized representative of the health institution where the child was delivered or where the child stays at the time of registration (Article 144 of the Family Code of Ukraine)

Parents of the child shall register the birth in the body of state registration of civil status acts without delay, if possible not later than one month after the child has been born (Article 144 of the Family Code of Ukraine).
Note: Late registration of the child's birth without a valid reason shall entail an imposition of a penalty ranging from one to three non-taxable citizens incomes (Article 212 of the Code on Administrative Offences of Ukraine).

Civil Status Act
State registration of the childs birth shall be conducted by the body of state registration of civil status acts together with establishing the parentage and assigning the family name, name and middle name and is attested with the birth certificate (Article 144 of the Family Code of Ukraine).
Rights and responsibilities of the mother, the father and the child are based on their affiliation to the child, such affiliation being attested by the body of state registration civil status acts according to the procedure prescribed by Articles 122 and 125 of the Family Code of Ukraine (Article 121 of the Family Code of Ukraine).
