A death certificate is an official document issued by the government, which declares cause of death, location of death, time of death and some other personal information about the deceased.
HOW TO APPLY
- An applicant should visit the National Identification Registration Authority (NIRA) with all the required documents and bank slip for the application fees
- Note : Visit any bank of your choice request for the account number of NIRA and deposit the application recommended fees
- Approach the service desk at the NIRA offices request for an application form , fill in all the information correctly and submit the filled form with the required documents to the attending official
- The official will then verify and process all the document, and will then inform you when to come and collect death certificate.
Required DocumentsEdit
- Certificate of cause of death which should be Filled and signed and stamped by the authorised authority:Certificate of cause of death
- A NIRA Death notification Form 12 complete and signed by the Notifier:Notification of death
- The applicant original national identification card
- A Police Report where Applicable.
- National ID of the Deceased.
- Bank slip of the application fees

InstructionsEdit
- A notification Record for a death that has occurred in a medical facility shall include a document duly signed and stamped by the Hospital- administrator or superintendent.
- For a death that has occurred in the community, a death notification Record shall include a document signed and stamped by the Division – Town Clerk, Sub county – Sub county Chief, and shall have the following particulars of the event:-
- Date of death.
- Time of death.
- Name of Deceased.
- Place of Death.
- Once the notification documents are verified to be true the authority will register the death of the deceased person and consequently issue a death certificate signed by the notifier of a deceased’s death.

- The National Identification and Registration Authority is the organization tasked to ensure registration of births and deaths and issue their certificates.
- One in pursuit of a certificate of death of a deceased has an obligation to first notify the authority of the death of any deceased person, pay a prescribed fee and obtain a death certificate.
