Procedure

1. Go to the Registrar of Births and Deaths office nearby the place where death occurred.
2. Register the death at the Deputy Registrar.
3. Fill in the Form 2 to register death and submit it.
4. The Deputy Registrar will enter the information in the Registrar of Death and on the duplicate sheet to be supplied to the Registrar at Apia under regulation.
5. Upon completing the entries in the Register of Deaths, the Deputy Registrar shall request, the informant, if then present, to sign the entries, but the informant shall not be obliged to sign the entries.

• The death of a person must be registered within 1 month after the date of death.

• Date of death

• Place of Buried

Cause of Death

• Cause of death and interval between onset and death
• Medical attendant by whom certified and date last seen alive
• When he last saw deceased

Parents

• Name and surname of father
• Occupation of father
• name and surname of mother

If Deceased

• Place of marriage
• Age of marriage
• To whom married
• Age of widow or widower if living
• Ages and sex of living
