Procedure

Download through UBDMIS Portal

1. Use the following link to access the portal : Link
2. Right side under the "Citizen Section" click on "Search Death Details"
3. Select the "Registration Unit" from the dropdown and enter the required information and click "Submit".
4. Information related to the death certificate will be displayed. Please check on the information and download the certificate.

Download Certificate through MeeSeva with Application Number

1. Please use the following link : link
2. Register yourself on the website if you are a new user Or you are an existing user you can login.
3. You can use your application number and search for status of the application and once the process is over, you can download the certificate.

Download with Application Number

NOTE:- All the details of death shall be forwarded by the hospital to GHMC within 7days of death.
After successful submission, the reference number will be sent to the registered mobile number (mobile number as per the information provided by the deceased family members to the hospital)

1. To Download or View Death Certificate, the applicant has to use the following link
2. Enter the Acknowledgement/Application number and click the “Search”.
3. Enter the OTP number sent on registered mobile number and click the “Verify OTP” button.
4. The Acknowledgement/Application number entered earlier would reflect.
5. Click on the “Submit” button and automatically the Death Certificate would get downloaded on your PC.
6. If there are any restrictions related to popup or downloads, the error msg would reflect.
7. The applicant can check and view the certificate from downloads.
8. Also, the applicant can print the death certificate by clicking the “Print” button.
9. To Download or View the death Certificate, the fee is free of cost.
10. The processing time for this procedure is within a minute.

Apply through MeeSeva Center

1. To Download or View Death Certificate, the applicant has to approach the nearest CSC link

   NOTE: To check the contact details of CSC centre, please select the District and Block.

2. The applicant has to visit the nearest centre.
3. The applicant has to inform all the details to the respective officer.
4. The officer will enter the details in the portal with the given details.
5. The applicant will get the downloaded certificate from the officer.
6. To Download or View the Death Certificate, the fee is free of cost.
7. The processing time for this procedure is within a minute.

Need for the Document

• A death certificate is either a legal document issued by a medical practitioner which states when a person died, or a document issued by a government civil registration office, that declares the date, location and cause of a person's death, as entered in an official register of deaths.

Information which might help

• Death certificate helps in closing bank accounts, claiming insurance, selling property of the deceased or any work related to banks. It is also required for property transfer or property tax payment. If the deceased is the owner of a property, then succession certificate will be required in the absence of a 'Will'.
